New Patient Information

We welcome new patients and look forward to helping you on your journey to pain relief. Before your first appointment, please have the following ready:

  • A valid photo ID and your insurance card
  • A current list of all medications you are taking
  • Any relevant medical records, imaging studies (MRI, CT, X-ray), or test results
  • A referral from your primary care physician, if required by your insurance

Insurance & Payment

Cellara Pain Institute accepts most major insurance plans. We recommend contacting our office prior to your visit to verify your coverage. Our team is happy to assist you with any insurance-related questions.

What to Expect at Your First Visit

Please arrive 15–30 minutes before your scheduled appointment to complete any remaining paperwork. During your consultation, our physician will perform a thorough evaluation, discuss your symptoms, review your medical history, and work with you to develop a personalized treatment plan.

Preparing for a Procedure

If you are scheduled for a procedure, our team will provide you with detailed pre-procedure instructions. General guidelines include:

  • Follow all fasting instructions provided by our office
  • Inform our team of any changes in your health or medications
  • Wear comfortable, loose-fitting clothing
