Understanding Vein Conditions

Varicose Veins

Varicose veins are enlarged, twisted veins visible beneath the surface of the skin — typically in the legs and thighs. They form when the valves inside the great saphenous vein or other leg veins fail, allowing blood to flow backward and pool.

Common symptoms:

  • Visibly bulging, rope-like veins under the skin
  • Leg aching, heaviness, or fatigue — particularly after standing
  • Swelling in the lower leg and ankle, especially in the evening
  • Throbbing, cramping, or itching around the vein
  • Skin changes around the ankle (darkening, thickening, or eczema-like changes)

Spider Veins

Spider veins (telangiectasias) are small, web-like clusters of dilated blood vessels visible just beneath the skin surface. They are typically red, blue, or purple, and most commonly appear on the legs and face. While usually smaller and less symptomatic than varicose veins, spider veins can cause aching, burning, or itching.

Chronic Venous Insufficiency (CVI)

Chronic venous insufficiency is the underlying condition driving both varicose veins and many cases of leg swelling, discomfort, and skin changes. It occurs when venous valve dysfunction causes persistent backward flow (reflux) and increased pressure in the leg veins.

Symptoms of CVI:

  • Chronic leg swelling (edema), especially in the lower leg and ankle
  • Heavy, tired, or aching legs
  • Skin discoloration or thickening around the ankle
  • Varicose veins
  • Itching or burning of the skin
  • Venous ulcers (open sores near the ankle) in advanced cases

CVI is diagnosed with a duplex ultrasound examination, which maps the venous anatomy and identifies reflux. Dr. Osman uses diagnostic ultrasound to properly characterize your vein disease before recommending treatment.

Leg Pain from Vein Conditions

Many patients are surprised to discover that chronic leg pain, heaviness, or aching is caused not by arthritis, back problems, or “normal aging” — but by treatable venous disease. Venous leg pain is typically a dull, heavy aching that builds throughout the day, worse with prolonged standing or sitting, and relieved by elevating the legs.

Our Vein Treatments

EVLT — Endovenous Laser Treatment

Endovenous Laser Treatment (EVLT) is a minimally invasive, office-based procedure that uses laser energy delivered from within the vein to close the diseased great saphenous vein or other incompetent truncal veins.

What to expect during the procedure:

  1. Duplex ultrasound maps the saphenous vein throughout the procedure
  2. A very thin laser fiber is inserted into the saphenous vein through a small puncture — no surgical incision required
  3. Local tumescent anesthesia is injected around the length of the vein
  4. The laser fiber is slowly withdrawn while delivering calibrated laser energy, sealing the vein shut
  5. The entire procedure typically takes 45–60 minutes per leg

Recovery: Compression stockings worn for 1–2 weeks. Walking is encouraged immediately. Most patients return to light daily activities the same day or the following day.

Insurance: EVLT is often covered by commercial PPO insurance when medical necessity criteria are met.

Radiofrequency Ablation (RFA) for Veins

Vein RFA uses radiofrequency energy to heat and close incompetent superficial veins from the inside. The ClosureFast procedure delivers controlled radiofrequency heat through a specially designed catheter to seal the saphenous vein.

What to expect: The RFA catheter is introduced through a small needle puncture under ultrasound guidance. Tumescent anesthesia is administered. The catheter is withdrawn in segments, delivering radiofrequency energy in controlled 20-second heating cycles. The procedure typically takes 30–60 minutes.

Recovery: Compression stockings for 1–2 weeks. Most patients resume normal activities within 24–48 hours.

Sclerotherapy

Sclerotherapy is the gold standard treatment for spider veins and small varicose veins. It involves the injection of a sclerosing solution directly into the target veins using a very fine needle. The treated vein collapses and is gradually reabsorbed by the body over several weeks.

Sclerotherapy is appropriate for:

  • Spider veins (telangiectasias) anywhere on the legs
  • Reticular veins (medium-sized blue feeder veins)
  • Small to medium varicose veins
  • Residual varicose veins following EVLT or RFA
  • Patients who want cosmetic improvement of visible leg veins

What to expect: Using a very fine needle, the sclerosing solution is injected directly into each target vein. Multiple veins can be treated in a single session — typically 20–40 individual injection sites per visit. Sessions typically last 30–45 minutes.

Recovery: Compression stockings for several days. Veins gradually fade over 4–8 weeks; multiple sessions may be needed for optimal results.

Spider Vein Treatment
