Interventional Pain Care

At Cellara Pain Institutewe take the time to understand and support your pain management needs. Each patient receives an individualized care plan based on comprehensive diagnostic evaluation. Treatment options may include referrals to physical therapy, minimally invasive image-guided interventional procedures, or medication management.

Our Harvard-trained physicians work closely with you to identify the precise cause of your pain and provide variety of evidence-based options to manage your chronic condition. We also collaborate with your primary care physician or referring provider to ensure continuity of care and to keep them updated on your treatment progress.

Preparing for Your New Patient Consultation at Cellara Pain Institute

Before your first visit, you will receive the following forms via email or mail:

  • Release of Medical Record Information

  • HIPAA Notice of Privacy Practices

  • Consultation Attendance Policy

  • New Patient Pain Consultation Form

Please complete and bring these forms with you to your appointment.

What to Bring:

  • current list of medications

  • All diagnostic imaging reports (MRI, CT scan, X-rays) related to the pain area

  • EMG/Nerve conduction study results

  • Relevant medical history, including past hospitalizations

  • Mental health records, if applicable

  • The last two progress notes from previous providers

  • Records of recent therapies (e.g., physical therapy, chiropractic care, injections)

  • Any surgical records related to your condition

What to Expect During Your Consultation:

Arrival & Check-In: Arrive 15–30 minutes before your scheduled appointment. A front desk staff member will verify your photo ID and insurance card.

 

Initial Screening: A clinical assistant will record your vital signs (blood pressure, weight, and height) and review your medication list. You will also be asked about your pain symptoms and referral source, if applicable.

 

Physician Evaluation: After a short wait, your symptoms will be reviewed, and your physician will perform a comprehensive physical exam. They will then discuss possible causes of your pain and work with you to finalize a personalized care plan.

 

After the Visit: Before leaving, we may discuss the insurance pre-authorization process for treatments, refer you for additional imaging or therapy, or schedule a follow-up appointment.

 

Communication with Providers: Clinical notes from your consultation and follow-ups will be shared with your referring physician or primary care provider.

Numbness and Tingling
